NOT ALL INSURANCE PLANS ALLOW DIRECT BILLING. PLEASE CHECK WITH YOUR INSURANCE COMPANY IF DIRECT BILLING IS ALLOWED.

School self-purchase insurance plans:
These are the plans offered at the beginning of the school year for purchase. Generally, you must use any extended health plans prior to utilizing the school plans and payment is out-of-pocket. A paid receipt must be submitted by the patient for reimbursement.

Athletic Associations or Teams:
